Consumer Health Data Privacy Notice

Golden Surrogacy LLC respects the sensitive nature of information about fertility, pregnancy, and family building. This notice explains how we handle consumer health data in connection with our inquiries, applications, and agency services where applicable consumer-health privacy laws protect that information. It supplements our Privacy Policy. Medical professionals and other independent providers may have their own notices and obligations.

Information and sources

Consumer health data can include information identifying someone as seeking reproductive or surrogacy services, even when it is not a medical record. Depending on your request and stage of engagement, the information we collect can include:

• Contact and identifying information connected with an inquiry about surrogacy, fertility, or pregnancy.
• Family-building interests, fertility-clinic information, pregnancy or delivery history, health-related eligibility responses, and information you choose to include in a message.
• Medical or screening information provided during onboarding or a journey, as authorized and relevant to the services requested.
• Website or communication information, such as an IP address, referring page, campaign information, or interaction with health-related content, when that information identifies or permits an inference about a person’s health or efforts to obtain health-related services.

Sources include you, the browser or device you use to interact with us, and people or professionals you authorize to provide information for your requested service. We ask you not to send complete medical records, Social Security numbers, or payment-card details through a general website message.

Purposes and processing

We collect and use relevant information to respond to inquiries, discuss programs, assess initial fit, coordinate authorized screening and journey steps, communicate with you, maintain related records, and handle privacy requests and applicable legal obligations. Golden coordinates agency services; qualified medical professionals determine medical suitability and clinical clearance.

Processing can include receiving, organizing, storing, retrieving, and communicating information through our forms, records, and communication systems. We collect or share consumer health data with the consent required by law or to the extent necessary to provide a service you request, as permitted by law. A website visit or acceptance of general Terms of Use does not provide blanket consent for unrelated uses of health information.

Disclosures and recipients

The categories of information described above may be disclosed as needed for the following purposes, subject to applicable permissions and legal requirements:

• Form, records, and communication support: Ninja Forms supports website inquiries. Intended Parent consultation requests and Surrogate applications pass through Zapier to Insightly. Microsoft 365 supports email, and GoTo Connect supports calls and SMS. Providers receive the information needed for those functions.
• Requested journey services: relevant medical, mental-health, legal, insurance, escrow, records, and other journey professionals, and prospective or matched journey participants when you authorize that disclosure for your requested service.
• Website functions: hosting, security, analytics, and embedded-media providers can receive technical or interaction information for the functions they support. These functions are separate from the systems used to receive and manage application information.
• Legal obligations: recipients of disclosures required or otherwise permitted by applicable law.

Golden does not sell consumer health data. No parent company, sister company, or other business under common ownership receives Golden’s lead, application, or medical information.

Website choices

Our website uses Google Analytics for visit measurement and includes YouTube videos. When these third-party services load, their collection can include information about your online activity. Their own practices and settings may allow information to be associated with activity over time or across websites and services. Review the cookie-preferences control and the relevant provider’s privacy notice when choosing optional services. Google Search Console provides website search-performance information; it is not the route used to deliver your application to Golden.

Requests, corrections, and appeals

Contact info@goldensurrogacy.com to submit a consumer-health privacy request. Depending on applicable law, you may ask us to confirm collection or disclosure, provide access, identify relevant recipients, delete information, withdraw consent, or stop collection or sharing. You may also ask us to correct information. State what you are requesting and how we can contact you. Do not include medical records in the initial request.

Our Director of Operations reviews requests, with advice from counsel when needed. We may request information reasonably needed to verify your identity and authority. We respond within the applicable legal timeframe, explain any permitted extension or refusal, and pass deletion requests to relevant recipients when required. You do not need to create a new account to make a request.

If we decline a request, you can appeal by replying to our response or emailing info@goldensurrogacy.com with the subject Privacy Appeal. Explain which decision you want reviewed. We will provide a written decision and reasons within the applicable timeframe. If an appeal is denied, we will provide the relevant state Attorney General’s complaint contact or method where required. We do not unlawfully discriminate against anyone for exercising privacy rights.

Changes and contact

We will post the current notice and its effective date here. If a material change requires additional notice or consent, we will provide that notice or obtain that consent before the changed collection, use, or sharing occurs. Questions may be sent to info@goldensurrogacy.com.
