Golden Surrogacy LLC operates this website. This notice explains the information collected through our website and inquiry forms, how it is used, and how to contact us about privacy. Separate notices or agreements may apply to clinical services, formal screening, and other services provided during a surrogacy journey.

Our Consumer Health Data Privacy Notice provides additional information about sensitive health-related information where applicable consumer-health privacy laws apply.

Information you provide

When you request a consultation, apply to become a Surrogate, or contact us, we collect the information you enter. Depending on the form, this can include your name, contact information, location, date of birth, citizenship, preferred language, relationship or family-building information, fertility-clinic information, referral details, program interests, eligibility responses, and any message you choose to provide.

Some information about fertility, pregnancy, or interest in surrogacy can be sensitive health-related information. Please provide only the information requested for the purpose of your inquiry. Do not place payment-card details, Social Security numbers, or complete medical records in a general website message.

Information collected by the website

Our forms and website services can receive technical information such as your IP address, browser or device information, referring page, and campaign information associated with your visit. The site also uses cookies or similar technologies for essential functions, preferences, security, and any optional services you allow.

How information is used

We use submitted information to respond to your request, discuss relevant programs, assess initial fit for the service you requested, coordinate next steps, maintain related records, and protect the website against misuse. Submitting a form does not constitute medical clearance, acceptance into a program, a match, or a signed service agreement.

Marketing communications and optional tracking are handled according to your choices and applicable requirements. A request for information does not authorize every possible use of your sensitive information.

Service providers and disclosures

Our website uses Ninja Forms to collect inquiries. Intended Parent consultation requests and Surrogate applications are routed through Zapier to our Insightly customer relationship management system so Golden can respond and manage next steps. We use Microsoft 365 for email correspondence and GoTo Connect for telephone calls and SMS communications. These providers process information needed to support those functions.

Website hosting, security, analytics, and other service providers may also process information for the functions they support. Medical, legal, insurance, escrow, and other journey professionals may receive information when needed for a service you request and as permitted by applicable law and required permissions. Relevant information may also be shared with prospective or matched journey participants when you authorize it for your requested service. We may disclose information when legally required or to address fraud, security incidents, or legal claims, as permitted by applicable law.

Golden does not sell the personal information it collects. No parent company, sister company, or other business under common ownership receives Golden’s lead, application, or medical information.

Cookies, analytics, and embedded video

We use Google Analytics to understand website visits and Google Search Console to understand the website’s visibility and performance in Google Search. These measurement tools serve different purposes from the systems used to receive and manage your consultation request or application.

You can review and change optional website cookie choices through the cookie-preferences control. Essential technologies support functions such as security and remembering privacy choices. Optional services may include analytics and embedded media. Loading a YouTube video or following an external link can involve another provider’s services and privacy practices.

For inquiry and application handling, functional first-party browser storage may remember your first referral source and campaign details for up to 30 days, independently of optional cookie choices, and include them with the form you submit.

Your browser may offer additional privacy controls. You may also contact us about your privacy choices and requests using the contact information below.

Security and retention

The website uses HTTPS to encrypt information in transit between your browser and the website. No website or information system can be guaranteed completely secure. Information held in other systems is subject to the safeguards and access arrangements applicable to those systems.

We retain inquiry, application, and journey-related records according to their purpose, any later onboarding or screening activity, and applicable legal requirements. Records that include medical information are retained according to the state and federal record-retention requirements that apply to those records. Before deleting information, we consider these requirements and any applicable contractual, accounting, or dispute-resolution obligations.

You may contact info@goldensurrogacy.com to ask about your information, request a correction, or submit a privacy request. Our Director of Operations reviews and responds to these requests, with advice from legal counsel when needed. Depending on your location and the applicable law, additional rights may include access, deletion, withdrawal of consent, or an appeal. We may need information to verify a request and may retain information when the law permits or requires it.
